Can't remember the exact details and I think this could have been commentary from the dvd, but in Pee-Wee's Big Adventure, a stuntman riding the bicycle nearly lost his head during the scene in the Hollywood lots. Can't remember why, or how he survived, but it's crazy to think how even a kids movie like can be dangerous business.

Also, you have to be genuinely insane to have your crew essentially mutiny right on the set. James Cameron is a psychopath to risk someone's life like that.
