Yeah it's very weird and I'm not sure I fully understand it yet. I think what happens is that when you click Add Friend on someone, you start following them. So at that point they WILL show up in your friends list. As far as I know though it doesn't give that person any warning at all that you added him. So they will only be friends "following you" if they decide to go to your name and click Add Friend.
