I use the add-on SaveFrom.net - helper for Firefox to download videos from YouTube and other places. Well, it contains the malware Offers4u. I'm on a mac and it took me disabling all add-ons one by one to figure it out since it wasn't installed anywhere. If you've seeing that pop-up, disable SaveFrom.net - helper and see if that fixes it. I contacted Mozilla about it.
