Cable tv sound/picture out of sync

I've had to hook my cable DVR directly back to the TV because the sound and picture keep falling out of sync when directed through the Xbone.
By like a full second at that.
I've searched Google but can't cone up with anything. Anyone else experience this?
 
I have DISH and I have an HDMI cable and an optical cable. With just using the speaker from the tv I get the delay but when I have the receiver on and use the big speakers/surround sound the sound is perfect. Don't know why. Have it all set to come on together so it's not a big deal to always use the speakers and receiver and the tv volume I just have turned all the way down.

It shouldn't if audio is coming from the same source as the video (HDMI). The delay should be the same. Generally, you see this kind of stuff if you run audio directly out of the source (i.e. cable box) into a sound system and then have the video only running through something else (Xbox One, etc.). I run my Xbox 360 running MCE through the X1 via HDMI and then have the X1 output optical to my soundbar. Generally, if you let the X1 output the audio (via HDMI or Optical), then there shouldn't be any delay....at least I do not experience any. There may be some exceptions due to compatibility with certain equipment, etc. Some sounds systems (receivers, soundbars) have delay settings built into them just for this purpose (i.e. to sync the video/audio if they are being processed in different "chains"). This is ok for TV and movies assuming you can get them in sync.
